Passport / ID card renewal for minors (under the age of 18)

General information

Documents to be submitted

  1. A fully completed and signed application form (one per document) to be found under 'Forms'
  2. All current Dutch documents (you must submit all documents)
    a. passport
    b. identity card
    c. Lost/stolen: a lost declaration form (C2) and if possible an old passport/identity card (find under forms )
  3. Two passport photographs according to specific Dutch requirements
    - Find a list of instructed photographers at 'Passport photographs' on this website
    - British and EU standards are different than the Dutch
  4. Recent proof of residence (eg. British driving license, bank statement, utility bill) - in name of parent(s)
  5. Original birth certificate of the child stating the names of the parents
    - If the child is born in a country where it is impossible to get a birth certificate (ie. Somalia, Iraq, Afghanistan) you will have to submit a full extract (ask for: uitgebreid uittreksel uit de GBA) with (both) parent(s) mentioned. You can obtain this extract from the last town hall (Gemeente) where you lived in the Netherlands.
  6. Parental responsibility:
    a. (photocopies of the) passports of both parents (holder- and children page)
    - a minor included in one or both of parents passports who now applies for his/her own passport must submit the original (child will be taken out of this passport)
    b. written permission of father and mother (or person(s) who has parental responsibility)
    - permission by signing the application form or submitting a written and signed letter (download from website)
    c. original marriage certificate/ divorce certificate/ custody papers/ death certificate/ declaration of acknowledgement ('akte van erkenning) / Dutch adoption papers
  7. For identity card applications: a certificate of de-registration ('uitschrijvingsbewijs') issued by the last municipality/town hall where you lived in the Netherlands
  8. Payment in cash; for current prices visit the website (cheques, debit/credit cards cannot be accepted)
